CDC has created the STEADI (Stopping Elderly Accidents, Deaths and Injuries) Initiative to help primary care providers prevent falls in their older population. STEADI includes a tool kit for health care providers to use to assess a patient’s risk for a fall and action items that can be done if the patient is found to be at a low, moderate or high fall risk.
